No representative of the data controller or data protection officer is provided for or appointed.
2. Purposes of processing
The data you provide for the requested service are processed solely for the following purposes:
Receiving, assessing and handling the compounding advice request
3. Categories of personal data
The personal data processed are those you provide for the tasks described under “Purposes of data processing”. Specifically, these include your name, contact details (e.g. email, telephone), request text and attached documents, which may contain health data. Providing these data through this channel is optional; you can always contact the pharmacy directly.
4. Recipients of personal data
No transfer of the data to any recipient is envisaged. No transfer of these data to third countries is envisaged. Personal data will not be made public.
5. Retention period and processing methods
Your enquiry and attachments are stored in the restricted laboratory database on Aruba hosting for corradinidelia.it. They are not published on the website or automatically forwarded by email. Access requires an authorised account: collaborators see enquiries and active orders, while archives and management functions are restricted to the administrator. Staff may contact you by email or WhatsApp according to your choice. Attachments of completed orders are deleted from the operational database 10 days after the completion date and time. The order and its history remain in the restricted archive. Active and cancelled orders are not covered by this automatic deletion; cancellation does not delete the data. External copies, original documents and files already downloaded are not deleted by this function. The pharmacy manages the retention of enquiries and backups for as long as needed for the consultation and any applicable obligations. Contact the controller for information or deletion requests.
6. Your rights
You have the right to request access to your data from the controller (Article 15 of the Regulation), rectification (Article 16), erasure (Article 17) and restriction of processing (Article 18). You also have the right to object to processing (Article 21) and to data portability (Article 20). You may lodge a complaint with the supervisory authority and withdraw your consent at any time, without affecting the lawfulness of processing carried out before withdrawal.
7. Provision of data
Providing the above data is mandatory for the purposes stated in section 2. Refusing to provide mandatory or necessary data may make it formally and practically impossible for the pharmacy to provide the requested service.
□ I declare that I have received the information above in accordance with Articles 13 and 14 of European Regulation 2016/679. As the data subject, I consent to the processing and disclosure of my data within the limits, for the purposes and for the period stated in this notice; in particular, I consent to the processing of the data whose provision is mandatory under point 7 of this notice.
